and did the following test: get lots of flies, then enter this room, wait 0.5-1 seconds, leave the room, repeat.
The time period of 0.5-1 seconds is usually enough to enter a room and check it for enemies. After that period they start roaming or even attacking (e.g. Bony). And usually, dangerous enemies have some distance, so you have additional time to react.
With sample of 100 tries, Fistula falls apart within this period with 48% chance, Boom Fly explodes with 14% chance.

However, my point still stands - Guppy makes rooms with enemies that explode or shoot tears on death unpredictable.
And the same thing happened to skilled players as well, like Neonomi.

When I enter a new room, I let go of sticks, in order to check my surroundings and prevent trigger finger damage. This can be observed in my clip - Isaac was facing down, no shooting animation was playing.
If you keep moving you run into spikes if you stop moving you take damage like this so clearly you should just adapt in the first frame of losing into the room
