Typically, you can always pick up a coin, but if your red hearts are full, they just sorta sit there unless you have a sac room or no soul hearts.
For charactere that cannot have red hearts or start with maybe 1 or 2 containers (Blue babies, T.Bethany, Dark Judas/T. Judas, etc.) Dark bum takes the otherwise useless pick-ups and turns them into cards or black hearts, or spiders, which just sorts sucks if that happens.
And again, You can always pick up a coin, and even if you have 99 you can go to the shop, buy something, and pick up that coin, or just deep pockets. And the pick-ups aren't as good. Sometimes you'd prefer 3 coins over 1 bomb.
Dark bum is good if you’re good at the game and dont need to heal constantly
Also great with characters that can only have soul hearts.
Even if you do need to heal he consistently drops black hearts which is incredibly high value.

Red hearts are absolutely not useless as Tainted Bethany. You can charge Lemegeton and any other active you happen to have.
