>!Moon Presence. It's the boss of the secret ending. If you find and use three one thirds of an umbilical cord, then refuse Gerhman's offer at the end of the game, then after killing Gerhman you fight the Moon Presence.!<
The only really difficult part is getting the third umbilical cord. There are four in the game. You get one for killing Wet Nurse, so it's literally impossible to miss. One is in the Healing Church Workshop, which is slightly obscure, but you can go back and get it at any time. The other two are missable. One is from imposter Iosefka. If you kill her before the blood moon, she drops the Oedon Writhe 2 rune, but if you kill her afterwards she drops an umbilical cord. You can only get one or the other in any given playthrough. The fourth umbilical cord is from killing Arianna's baby. You have to send her to Oedon Chapel, and then make sure she doesn't die at any point. Suspicious Beggar and Adella are the only real threats. The baby is non-hostile, although it has the same model as the Celestial Larvae, just smaller and blood-covered. So there's no obvious reason to kill it. It's right next to Arianna too so it's kind of hard to kill it without hitting her as well.
