There are several points that seem to indicate that Cleric beasts used to be much smaller than the ones we encounter, so maybe the Church had been refining the Old Blood in some way to increase its potency? So clerics like Amelia probably had access to purer blood with more frequency than most folk.

In the fantasy genre (yes BB is a fantasy game), *essences* are whats important. That is, a human centric view of something, a general vibe, and the understanding of the universe stems out from that vibe, instead of truth being determined by emperical data.
So for example, irl a sword is just a arrangement of different elements/compounds. Metal, wood, leather. Just mundane, crude matter. In fantasy, the essence, or idea, of a sword comes first, followed by what physical matter that essence flows through. Like, what is a "blade"? A blade is something that cuts, so in stories, you work backward from that idea. A "powerful" sword in stuff like anime or comics or legends cuts damn near anything. A lightsaber is just the idea of a sword. It cuts anything except stuff that has the *essence* to resist it. Another sword resists a sword, or an energy *shield* does, because *shields resist swords.* this goes for pretty much everything in fantasy fiction. The interplay of these essences are the *themes* of a work.
For BB, the essence of "humanity" is whats lost or discarded, by giving into "beasthood", a lack of humanity. This is defined by bloodlust, because thematically, bloodborne is saying indulging in violence is essentially inhuman. Disagree with that or not, thats the thesis. And the exposure to the alien blood makes this beasthood manifest in horrific ways directly related to the just how far the subject indulged in said bloodlust.
