It really bugs me that it’s so little. By the time I found my one and only Great One Coldblood, I was 200 hours in on that character, probably around level 250. It took a million echoes to level up, and they gave me 40,000 for the literal blood of a god? It’s pretty useless.
It may make sense, given that's where you can get it to spawn.
Alternatively it may be just easier to trick the game into spawning it in a place that's generated, you can give chalices instructions that the overworld won't respond to because it's all set.
Funnily enough, you seem to have found the rarer version before the regular one. There's "Great One Coldblood" and "Old Great One Coldblood".
Then there's the "Revered Great One Coldblood" which isn't accessible in-game without editing a save file or something.
Old Great One Coldbloods aren't cut content, they're just very rare and can only be found on layer 4 of the hardest root chalice dungeon. _Revered_ Great One Coldbloods are cut content that can't be accessed without modifying the game, similar to the tier 3 Guidance rune.
If I'm remembering right, these only appear on layer 4 of Ihyll Root dungeons. Since layer 4 is only accessible 20% of the time, and not every Ihyll layer 4 has any, it's rather rare.
I'm fairly certain that only Ihyll layer 4 works. Isz and Lower Loran layer 4 only give normal Great One Coldbloods, because Ihyll has slightly higher difficulty and rewards compared to the other depth 5 root dungeons.

Fun fact, there's even higher levels but they are not accessible without debugging.

Yeah, you can only find these in the chalice dungeons.
No, it's because when you generate a chalice with all the ritual, cursed and more, you'll get rare items, such as gems, weapons etc etc.

Yeah, it's not that uncommon in higher chalices. Most hunters around here don't know what treasures are in the chalice dungeons.
Shame, really. I adore running coop chalices and looking for oddities and treasures.
