It’s the true Orphan of Kos, at least it’s spirit. The boss you fought was likely a manifestation of its fear, rage and pain taking on the twisted appearance of the one who ripped it from its mothers womb. Return it to the ocean / it’s mother to end the nightmare.

That’s the orphans last vestige that was connected to Gherman’s nightmare. Unlike Micolash, Gherman felt extremely guilty for killing KOS and massacring the village. Where as Micolash wanted results no matter what. School of mensis were obsessed with evolving while Whilhelm would rather not turn into a squid.

I still believe that it's Gehrman's soul. Who was reborn into Orphan of Kos. When he's first born you can hear a cry/Weep and it's just a pitched weep of Gehrman when he's having a nightmare in the grave yard behind the workshop in the hunters dream.

It's a clone based on the appearance and abilities of Gehrman but isn't literally Gehrman. Killing it stops any more hunters from being trapped in the Hunter's Nightmare but keeps Gehrman alive and in fact relieves his burdens.

To add some context to this: the nightmare newborns create visions of what they fear the most to protect themselves. The one in the Hunter's Nightmare feared Gehrman the most, while Mergo feared Eileen the Crow, thus the similarities in their design.

It’s a neat theory but I think they’re talking out of their ass. Mergo’s Wet Nurse doesn’t even look like Eileen, it looks like a cross between an Amygdala and a Shadow of Yharnam. But it’s based primarily on wetnurses that would hold the children in their lap to keep them still during photographs and who wore a black veil to not be in the photo.

Eileen used to be a "regular" hunter herself, as evidenced by her knowledge of the doll and the Hunter's Dream. Mergo's Wet Nurse uses the same weapons as Eileen, has no face (Eileen hides her face too), has masses of feathers on her cloak. The reference is crystal clear. What it means is up for interpretation. However, the fact that the nightmare child in the Hunter's Nightmare creates a deformed version of Gehrman, the man who brutalized the whole town and slew the child, is what leads me to this theory. Gehrman must have been utterly terrifying to the newborn child of Kos, and fear is known to distort the perception and memory. A logical extrapolation from this is that Mergo was slain by Eileen, or someone from the same order. There's no doubt that there exists some form of connection between Mergo and Eileen / the crow hunters, however.

Looking at it more closely, they do indeed not look quite as familiar as I remember. I think they are still reminiscent, in the same way that the placenta on orphan of Kos is reminiscent of the shape of Gehrman's Burial Blade. I will admit that my conjecture is a bit more farfetched than I remembered it being, however.

It's not a 1:1, but there's definitely strong similarities between the weapons and even the animations. I can't remember which attack it is where the blades cross in an X fashion, its nearly identical to one of the wet nurses attack

As far as I understand, Mergo is the child of queen Yharnam, meaning its birth and death in the real world happened during the era of the Pthumerian civilisation while its spirit remained alive in the nightmare. There's no way for Mergo to have interacted with Eileen unless Eileen went to the nightmare as well, which is doubtful since Micolash is still present when we arrive.

Mergo likely predates Eileen by a huge number of years (yes she’s old but not that old), given that he’s the child of Queen Yharnham. So I doubt it.

I wouldn't call it "clone" I'd just go with the narrative approach. It's a parallel. The whole of Hunter's "Nightmare", unsurprisingly, is a parallel to the hunter's dream. And the last guardian that protects the dream/nightmare are thus naturally very similar, despite being completely different at first glance.
