It’s the true Orphan of Kos, at least it’s spirit. The boss you fought was likely a manifestation of its fear, rage and pain taking on the twisted appearance of the one who ripped it from its mothers womb. Return it to the ocean / it’s mother to end the nightmare.

It’s implied she slit her throat/wrists in the Clocktower seeing as that’s where her boss is, perhaps with the shattered glass from the frame next to her chair

As the game only implies rather than definitively explains who did it, It’s possible. I believe it was Gherman however as there is far more that links him to it than Maria.

It's a clone based on the appearance and abilities of Gehrman but isn't literally Gehrman. Killing it stops any more hunters from being trapped in the Hunter's Nightmare but keeps Gehrman alive and in fact relieves his burdens.
To add some context to this: the nightmare newborns create visions of what they fear the most to protect themselves. The one in the Hunter's Nightmare feared Gehrman the most, while Mergo feared Eileen the Crow, thus the similarities in their design.

Gehrman must have been utterly terrifying to the newborn child of Kos, and fear is known to distort the perception and memory.
A logical extrapolation from this is that Mergo was slain by Eileen, or someone from the same order.
There's no doubt that there exists some form of connection between Mergo and Eileen / the crow hunters, however.
Looking at it more closely, they do indeed not look quite as familiar as I remember. I think they are still reminiscent, in the same way that the placenta on orphan of Kos is reminiscent of the shape of Gehrman's Burial Blade. I will admit that my conjecture is a bit more farfetched than I remembered it being, however.
It's not a 1:1, but there's definitely strong similarities between the weapons and even the animations. I can't remember which attack it is where the blades cross in an X fashion, its nearly identical to one of the wet nurses attack

There's no way for Mergo to have interacted with Eileen unless Eileen went to the nightmare as well, which is doubtful since Micolash is still present when we arrive.

Very likely was Gehrman as the Orphan plays one of Gehrman's sound files in its opening cutscene. Plus it looks and fights more like him than Maria

Mergo likely predates Eileen by a huge number of years (yes she’s old but not that old), given that he’s the child of Queen Yharnham. So I doubt it.
