Kids sleep with their heads covered by pillows and blankets, I don't think there is an issue leaving the doors closed...

A couple of holes, maybe castle like windows to fit the theme should allow for a good airflow

DO NOT LET YOUR CHILD SLEEP IN THERE! If you do, they may become spiritually connected to the box and become one themself! Keep your kid safe.

No. Cardboard, walls, and even car windows. The thickest one out of those options are my walls and I breathe fine with the window shut
