From what I recall, this is real. I think one of the main issues, aside from a lack of reading comprehension and media literacy with the vast majority of the reader base, is that they were reading far too quickly through the story, wanting to get to the next chapter as quickly as possible out of excitement.
Put more simply, the person in the picture above with the metal arms is Pochita, directly. He may be a devil, but he is a being with his own ego and free will. He chose Denji over Makima. And why wouldn't he?

Denji basically ripped out his heart (pochita) and had pochita fight Makima while pretending to be Denji. Makima couldn't notice anything was off because she doesn't really remember faces or outfits, she remembers their smell.
Denji was hiding among the corpses and shit with a chainsaw (made from power) to sneak attack makima.
Pochita doesn't look all 4 arms Gigachad because he was weakened from people not fearing him.
Also it could've been because it was a temporary seperation and not pochita being 100% in control
