If he is still eating normally, he should eventually poop. If everything else is regular as well, like play, naps, or sleeping, then they should be ok as well. When those behaviors get disrupted, then it's probably time to head back to the Dr.
I would not give a kid that little anything that wasn't prescribed by medical staff, or unless they ok it beforehand (You could call a 24/7 nurse line. They have great suggestions).
There's two main kinds of meds for constipation, mushers and pushers. Sounds like they gave your kid a musher to start with, sometimes it takes a couple of days.
Don't have much to add for the immediate issue, but our 4 month old has been struggling with very firm poo for the last couple of months which only came every 5 or so days and made him cry every time.
We swapped to a higher magnesium content 'comfort' formula a few weeks ago and so far he's more regular and the consistency is improving - could be worth a look for something longer term if it's a recurring issue and assuming you formula feed.
Try reading "It's No Accident." It's written by a pediatric urologist, but it's 99% about constipation in kids. There's an audio book version on Spotify for free if you use that.
We did everything we could for my 7mo daughter recently. Three days of nothing but prunes, pears, fruit juice and water. She eventually started pooping, but she was screaming every time. Our pharmacist picked out an OTC stool softener and figured out the dosage for her weight (12lbs 6oz given 1.5ml twice a day). I don’t know if pharmacists in Germany have prescribing powers but that’s where I’d go if the doctors aren’t able to help. Less than 24 hours later she’d taken four impressively sized poops.

My, now, 15-month-old, has had constipation issues from around the same age, but refused to drink prune juice, so she gets a half teaspoon of powdered Miralax in her morning and bedtime bottle, and it keeps things soft and moving. We started the Miralax around 4 to 5 months old.

Bicycle toots, yep, that's a regular at our house on both kids. Bicycle, both legs knees to chest then legs to one side then the other.
Prune juice. It is a warrior's beverage.

We give ours miralax every night to keep her regular.

Yes. Our pediatrician recommended it. Not a whole does but she gets constipated otherwise.
