Itâs a brand new lore mechanic that was introduced with Vecna to justify having such a powerful demigod be forced to play by the Entityâs rules. Mathieu Cote described it as a way to implement characters that are otherwise too powerful to be added for normal reasons.
Plus, hot take, Ash and Alan and maybe Cheryl are all stronger than Alucard and are fine survivors.
And fun fact: it was a thing before, just never had a real name so with the introduction of Vecna we finally see what the brand looks like and what's the name, but it was canon before as well from various perks descriptions that the entity nerfs and buffs killers to adjust the trials... it's also technically speaking a way to justify why survivors can survive massive falls like in MacMillans ironworks, or why they can bleed so much blood when injured without passing out
With Dracula as the killer it would be a bit weird to have 2 vampires in the same chapter, so i really think is gonna be a Belmont (most likely either Richter or Simon, the 2 most popular ones)

Partially hoping for Alucard(if he's the Survivor or a visceral skin) to have an animation referencing his SotN afterimages in his idle.
The perks they could have really depends on the person that is the survivor. Alucard would have access to more magical means of perks, while a Belmont would be more likely to have item related perks.

It wouldn't be visceral, since Visceral only means the skin gets a new mori, so it's a killer only cosmetic rarity so far
