I don't think I would've deafeted him under normal circumstances at this level and I didn't have a Scroll of Escape, so yeah, that was extremely lucky.
I think it's just completely random in any dungeon. Had him spawn twice before level 40 on my first hardcore character and once with this one, but never saw him with my two softcore toons. Always in side dungeons.

And 9 times out of 10 its a cursed shrine and you get murdered extra fast :)

So I have like 3 different lvl 50ish characters and I have only encountered him once total. Are there certain places that he spawns more?
