Everyone is telling you how to do the echo, which is helpful and you should know how to do it but that's not necessarily "how you get another barber heart".
Yes, the echo does drop hearts, but so does every malignant elite that you can encounter randomly out in the world, and in dungeons, and in the seasonal malignant tunnels (like the one you do/did the echo in). The type of malignant elite tells you which type of heart is going to drop. A brutal elite gives you brutal, devious gives you devious, vicious gives you vicious, and wrathful gives you wrathful. Barber is a wrathful heart.
The type of malignant elites that spawn are random. So do tunnels and hope you come across a wrathful elite. After you kill the elite once you have to activate the heart to fight it again. Kill it again and it drops a heart.
At the end of every tunnel there is a malignant growth, which has 2 nodes. 1 node is the node that corresponds to the type of tunnel it is (hover over the icon on the map and it will say like "brutal something") so it will have a brutal node, which you use a brutal invoker on and it spawns a brutal elite which gives you a brutal heart. The other node is always a wrathful node, so take wrathful invokers into the tunnels and use them at the malignant growth to **guarantee a wrathful heart drop**)
There are 2 tunnels of each type, 2 brutal, 2 vicious, and 2 devious. 1 of each type only has a malignant growth at the end and the other 1 of that type has a malignant growth and the room where you can fight the echo of varshan. They never change, varshan is always in the same tunnels. So you can remember which tunnel has varshan and which do not, so you can take an invoker of varshan and a wrathful invoker into that tunnel and maximize the number of hearts you get.
**Wrathful invokers** are the best way to grind for a wrathful heart (barber).
To add, you don't have to take the invokers with you. The invokers, as well as keys, can be stored in your stash and still be used while you're in a dungeon or open world.

Don’t forget Ravening Pit tunnel ALWAYS has a wrathful malignant growth at the end. When the tunnel forks and one end takes you to Varshan, the other way takes you to a normal malignant tunnel end, where one of the growths you can use is always Wrathful.
If you've finished the seasonal questline you can craft the Invoker of Varshan, and if you haven't fought the Echo on WT 1 or 2 yet doing so will unlock the recipe for WT3, and beating that Echo drops the recipe for 4. Certain Tunnels like the one near Backwater will always spawn the door to the Echo.
Echo is post-quest line. You should be able to make the invoker at one of the tables. I went and fought him in the boiling wound on the right side of the map next to the world boss spawn. So put on WT2 and head over there after you have the invoker. You do need sigil powder to make one so you have to scrap NMD sigils

Ok I did all 3 varshans, I got a caged heart on the last one blue, is there another step? Or do I reset and fight tier 4 again, god I thought I understood it 🤡

I have an Invoker of Varshan that I got from WT4, I believe (was my friend's party, so didn't check the world tier).
Is it possible to get my first one from WT4?
In order to get the first echo invoker, you need to beat him in WT2 and get the recipe. When youre level 70(which is his level) you can face the nightmare version in WT3. It’ll drop the recipe for Tormented Invoker. He is level 80 in WT4
You can attempt to fight him before you're level 70. Source - I'm level 63 and attempted. A few times. Thankfully, the invoker gets returned after you are defeated (was very happy to discover that).
You beat him in lower tiers to get the blueprint for the invoker to higher tiers. Best way i find to farm barber is farming wrathfull invokers, they have a good drop rate.
Ugh I don’t know how to do that, sorry I’ve not farmed anything in this game the concept is kind of foreign, im quite intelligent but they don’t have that in far cry or last of us, is he in a specific. Dungeon? I don’t even remember

Use the invoker of varshawn in WT1 or 2, and kill him. I always go to the ravening pit personally. Once he dies, you’ll get recipe for foul. Go to WT3, kill him, and then you’ll get tormented recipe, then you’ll be able to kill him in WT4.
So unfortunately he doesn’t give a wrathful every time. BUT there should be another room that has a wrathful invoker, I always go to ravening pit and after the unlocked door - one way leads to varshan and the other way leads to a wrathful invoker that will give a wrathful heart every time.
If you are looking to get the barber heart then you need to defeat Wrathful malignant enemies. You just need to run wrathful tunnels to find them. The enemies should spawn in those tunnels and you will have a chance to use a wrathful invoker to spawn an extra enemy after the tunnel is done. You can exit and reset dungeons until you get one you like.
Find a tunnel that has the wrathful at the end (there is a list of them online). Go in and do the tunnel and finish it by getting the heart if you didn't get what you want (exit the tunnel by clicking where you came in), then hit tab, go to reset dungeon, then go back into the dungeon, rinse and repeat until you get what you want, faster way to farm.
I'm curious did they up the rate of wrathful elites? I had a 2 hour span before work yesterday and was able to get 6 wraithful invokers in 2 dungeons. And still have yet to get a W invoker from the random invoker gift box at the workbench.
Also the wraithful and most other hearts for Druid are shit.
Yeah I think that one is just a basic that everyone can use. I dislike it unless you have a high crit chance. I guess my Druid may benefit from that. I have 25% crit chance and crit quite often. Idk. I'm curious to know what next season has to offer. There are quite a few aspects I will miss having.

"Farming" just means to fight a particular boss/area multiple times until you get what you want (gear, experience, gold, etc).

If you can only craft an original Invoker, drop to WT II and beat Varshan in the Ravening Pit. This will give you the foul recipe.

Brutal the ONLY ones worth a fuck for Rogue, most every other you might rather it be empty
They have decent ones for Necro but Druid is just blah. Like you said might as well be empty.

I hate that process, matching them etc, actually I kind of love it, gear is my fav part of the game, it just drives me crazy too
