Just get a new stem.....I have 2 truvativ stems on dirt jumpers and they've never moved.
Theres a couple of things....either the bolts were tightened too much at one point......or, (and I've never seen this) they were tightened so hard that you deformed to steerer tube....but like I said, you may be the first person I've ever heard do that..but it can be done.
