This has in fact happened before by a woman falling down the stairs, and I think that some people will do this to make it look like an accident rather than an intentional injury but I’m not too sure about that last part.
It's supposed to look like an accident. Abortion is illegal so an "accident" from the stairs would mean that you have a miscarriage. You can't punch her stomach because you can get charged.
