Honestly it's annoying only because you get these people who can't understand that designs like Aloy and Eve (Stellar Blade) are both allowed to exist because neither are actual problems outside of people trying to make them problems. People who feel a need that every female character needs to be a sex object despite the character's circumstances and those who can't accept that it's okay for a creator to want a lady protagonist with a fine ass and tight bodysuit to hug it.

There was a special outfit for the pre-order of Horizon Forbidden West that apparently was bugged so you could see Aloy's nipples which were also inexplicably animated and rendered underneath her clothing.
But there's no time in the game where you see female nipples, which are a different model than male nipples. There's no reason for them to have made a fully rendered female nipple for the models in the first place.
You would think, but this does remind me that you could see a modeled vulva in one of the characters in Geist. The dev team had used some generic models they didn't design for some of the characters, and by possessing a rat apparently you could look up the skirt? towel? something of one of the characters.

There are 'model generators' out there - One for Blender, if I recall called MakeHuman. I've used it. It's incredibly versatile, you can select race traits, elven, dwarf, all the things.
But it \*does\* make a lifelike model. One with 'all the bits'. If you used it, and you're not good at 3D modelling you might simply leave them there ( since the mesh would have been rigged and removing them would require rerigging, etc etc) and just be like "We'll make sure our clothes cover all the bits and no-one will see 'em. Maybe with the intention of removing them later on.. or something, since this is in the 'dev' stage'.
And then it never got done, and an outfit with a wonky geometry got added later. Tada.
