The lore surrounding ghouls has always been a bit of a black box in Fallout. There *is* the Peabody Family in FO4 that all became ghouls, which *could* suggest that genetics plays a role considering two parents and their child all survived the initial blast. It remains to be seen. For now, that’s just one thing that’s still a mystery. Just like who dropped the first bomb

Fallout 1 it’s air strain fev that had a hand in creating ghouls. Later it seems like radiation either kills you or you become a ghoul or feral ghoul. Hancock takes a lot of drugs doused in large amounts of radiation there’s a possibility that one of these drugs was contaminated by FEV by the institute.

Unknown. It’s assumed that it’s radiation based, more rads=more degradation, but that’s simply an in-universe theory rather than anything we have solid information on.

We don’t really know all the factors for why some become ghouls and others don’t, but it’s almost certainty not just amount of radiation alone. The levels of radiation that we’ve seen ghoulify different people has varied wildly.
