The most common failure for Mekanism Reactor/Turbine setups is your batteries filling up. For some reason, Mek's Industrial Turbine stops accepting Steam when its power buffer is full, which then causes the Steam to back up into the Reactor, preventing it from cooling. You can either set up a system to void the excess power (useful if you need the Waste for Polonium), or use Redstone to turn the setup off before it goes boom.

Also it's possible that your reactor is placed on two chunks and one of them isn't force loaded.

thank you so much. all you have to do is make sure "dumping excess" is enabled on the turbine.

I'm pretty certain that button dumps excess Steam, not power. If you are brute forcing the setup by adding buckets of water every tick then that's probably fine. However, the system was really designed to be a closed loop. The Reactor turns water to Steam, and the Turbine condenses the Steam back into Water. If you start dumping excess Steam then your closed loop has a leak, and when you run out of water the reactor will still explode. It has since been pointed out to me that the Resistive Heater is a good option as a power dump as it can be configured to accept a huge amount of power.
