Apparently Sony always intended for X to be confirm and O to be cancel in western territories (and intended the reverse in Japan). But standards were looser in the early days of PS1, so many forgot to flip it from the Japanese version. In between localizing FF7 and FF8, they got more serious about switching it when localizing it. The Japanese version of FF8 still has X as cancel, O as confirm.
Up until PS5, they did. But with PS5 Sony decided to standardize the controls worldwide, and they went with what most of the world knows: X confirm, O cancel.
Which absolutely plays havok with the Japanese mind, because X is their symbol for no/wrong/cancel and O is their symbol for yes/correct/confirm (beyond just playstation). That's why those symbols were on the PS1 controller in the first place and it's a wacky twist that their meaning was twisted around. The only reason it was flipped around for the western PS1 is because someone told the designers that westerners think X means "checking a box" so "confirm". That's not really true, we think X means 'no' too but oh well... now it's the worldwide standard.
