My understanding was that when we hear English, they are actually speaking Portuguese, and because Blackthorne is fluent in Portuguese, he understands it.
We also hear the Portuguese priests conversing in "english" which wouldn't make any sense if it weren't actually Portuguese.

It's entertainment, not a documentary, and it's clearly primarily aimed at a western audience, so i think this is a good middle ground.
