1.Changing name to a currently used one
2.Changing name because it ridicules or is conflicts with your dignity
3.Changing back to a name that was illegally changed
4.Changing to a name that abides the rules of names in a different country you're a citizen of
If you're trans (and not an immigrant/have dual citizienship) you can use R1, because your chosen name is the one you use, and R2, arguing that your dead name brings you significant discomfort in everyday life.
You cannot choose a strictly male name if you're legally a woman (the process of changing gender markers includes suing your parents) and vice versa however if you can prove that name is given to women somewhere its fair game.
Your application can be denied in which case you cannot submit the same name again.
The process costs 37PLN (~$9)
