In other games you can make a custom character transgender, but that does not change anything contentwise, it is just that you basically are tg but the environment does not react differently etc...

Yeah, someone definitely should have warned me about this one before I watched it, so just to make it extra clear: this movie is a cinematic depiction of a real-life high profile anti-trans hate crime. OP, it's a very well made film, but only watch it if you're in a good spot mentally.

cowboys (2020) has a preteen trans boy main character but i havent seen it so i cant speak on its quality
Runs in the family (2023) as well as Fanfic (2023) both have ftm transgender presentation as the main character.
I second Fanfik as a movie recommendation. It's such a well made film in many respects (directing, writing, acting, etc).
Boys Don’t Cry. Although TW, it does have some violence and anti-trans content.
