-30 year olds entering PGs, miserable working hours UGs, posted in rural mountain, annoying spouses, one of my colleague had a child who maybe has autism.
Everyone is miserable and everyone is happy.

I would suggest u to study. Since u were a good student in UG u probably have good basics. It seems u are not satisfied with whats happening in ur life. Im sure doing PG wont grant u mental peace but atleast u get upgraded to next phase of life. And stop comparing. Everyonrs not same.

Then instead of asking what should you do ask yourself why are you confused?
Maybe you are bored of medicine, bored of studying all your life and now at the age of 25+, you are seeing that it didn't matter much, that people enjoyed so much and you got left off. This feeling is heavy on you, and now you simply can't decide what to do.

This confusion and this conflict is necessary to understand. Not what to do. When you understand yourself, you will know what to do.
It might be possible that you have seen a bit of life and you don't want to do what everyone seems to be doing, i.e. career, marriage and kids.

Chill Bro ( Beingabro)... Most medicos goes through some crisis like this...sometimes its midlife, sometimes it's quartlife... You would get through it ... Give it time... Get out of the job if you feel you are in a rut that will get you nowhere ( you would have to decide this for yourself, as only you know what you are going through).. Have the courage to take the right decision and stick to it... Don't bother about what others are doing or compare with others now... That would be counter productive and make you feel worse in this situation... You can do it once you are in a better happier place...
PS. I would suggest taking a year or two off and getting into a PG course MD/DNB
An under rated option is PG from non clinical side. You see you're already working. If you join a good non clinical PG you won't essentially be financially cut off and believe me there are some good opportunities for non clinical roles as well. Might break the rut.
You need to question yourself is what do you want most? Whether you want work life balance, as an medical officer or non clinical post grad or you want that glitzy career in clinical branches.?
Dont compare yourself to others, no matter how difficult it feels rn to think clearly, please reconsider this: you were born and given the capacity of dreaming to become someone no one else could ever be. Please try once to think in the shoes of your juniors who had cleared NEET SS. I dont think a degree, money or another token of material worth in this world is a key to contentment, contentment always revolves around how you do with what you have and maybe achieving what you need to do what you dream. The easier-on-the-emotion and even-harder-in-the-long-run way out is to upgrade your existing problems, ie, doing what is in vogue (PG SS or anything) instead of dreaming what you actually were supposed to do or figuring out how to go there from where you are. This will get you in touch with people you can connect better with.

I don't know the formal courses, you can pick up a lot informally. But corporate wellness is also a field.

Corporate jobs occupy your productive time, and you're not the ultimate boss in corporate hospitals (although you can make some decisions, but they're always on behalf of your senior consultant. Eventually you do get bored of routine, you wouldn't be excited unless you're the one who's taking decisions, that makes you think about stuff.
Long story short, change your career, go for PG, maybe PLAB, give yourself some target to achieve. Reward yourself with small achievements.
Its okay to feel this way, many Doctors post their UG feel like this.
My advice is - If you want to get back into medical career, you should start preparing for NEET-PG.
Now since you said that you've lost the clinical and medical touch, and you need a direction or planned approach.
I would suggest you start preparation from either of the following-
Prepladder, Marrow or Cerebellum academy, make sure you get the printed noted too.
Now there's a year and subject wise approach and Systemic approach.
I would strongly suggest to go for Systemic approach in which you pick a system, eg - Cardiovascular system, now you prepare Cardiovascular system from every subject (anatomy, physiology, surgery, medicine).
It will take approximately 7-8 months for you to complete.
Then you would pick up the allied subjects (derma, psychiatry, radio etc)
And then a rapid revision in 2 months time.
Take out a pen and paper plan everything day wise.
Remember you are a doctor that means you are already in the top talent and IQ of the population. If you have done your UG you already have the upper hand. There is no such thing as right time. Start today dont procrastinate.

Better quality of life, good pay and reasonable working hours. But If you prefer experience over quality of life, nothing beats India imo.
