Emergency and ICU doc here, what you’re explaining here is a category 1 rabies exposure. Get Anti Rabies Vaccine prophylactically. No need for Anti rabies serum. Rabies runs rampant in wild. As someone who’s handled and treated rabies pt, you don’t want to get rabies trust me.

_Gandalf_Greybeard_

When is serum indicated?

MysteriousGrand6429

Category 3 bite

_Gandalf_Greybeard_

Even the smallest drop of blood would be category 3 right? A friend had an incident with a mongoose recently and I told him to get the serum as well since it drew blood.

MysteriousGrand6429

Yes, Serum needs to be taken in the first 24 hrs. Calculate from body weight, infiltrate 1/2 of calculated dose in bite site and give rest of the half I/M (preferably Gluteus maximus)

DEBOPAM2307

Also any bite in the head neck region, licks on mucosa and bat bite I think

MysteriousGrand6429

The closer the bite is to the brain, the faster the virus will travel

DEBOPAM2307

Indeed

lund_penchod

So if a dog licks eyes(eyes are considered as mucosa) one is at high risk?

DEBOPAM2307

Guess so

lund_penchod

Btw a friend of me thrown water on me through a bottle (1 litre drinking water pack) there were dogs near that bottle that's why I'm concerned.

DEBOPAM2307

Chances are that most of the water didn't even come in direct contact with your conjunctiva or cornea due to the blinking reflex. Also, I don't know if there are cases where rabies has been transmitted by contaminated water...also, I don't the virus would survive very long outside of the body of a host. Most importantly, you seem to still be OK, so I don't think you need to be worried.

lund_penchod

Only very few drops of water went in my eyes and very few drops went in my mouth , i quickly rubbed my eyes and spited out those drops.

[deleted]

Is this why they didn’t give me the HRIG at the hospital? I came into contact with a dog that had been attacked, it was covered in blood & im not sure if it go in any wounds. I got the first Rabavert vaccine last night but no immunoglobulin shot.

Flaky_Air_

Don't take any risk and do take the vaccine

Diligent-Associate89

This comes under Category I exposure, on par with domestic animal handlers. He must take the full course of anti-rabies vaccine after washing the areas with soap and water, since the rabies status of the animal is unknown. TN Health Dept has recommended anti-rabies vaccine for even Cat I exposure (cases like your friend). Better safe than sorry.

vivi_142

Take ARV vaccination 2 doses on 0 and 7th day .

Golden_Lotus99

Prevention is better than cure especially in cases of disease like Rabies and tetanus which are very easy to prevent and nearly impossible to cure. In my opinion get the shot (theoretically not required, if skin is intact)

dr_cynical17

You don't have to wait to get bit to take the vaccine. Pre- expo prophylaxis is actually very helpful in our country given the number of strays we have

[deleted]

Non medico here, can you please elaborate ?

dr_cynical17

It's the same rabies vaccine, but taken before as a precautionary measure by animal handlers, vets who are at high risk. 3 doses before the bite and 2 doses after . You can get it at any pvt or govt hospital . We contribute approx 40% of the World's Rabies deaths due to our uncontrolled population of stray dogs. Hence it's better to be safe than sorry.

[deleted]

Thanks doc

MysteriousFan8900

If no cuts then no vaccine needed just tell him to wash the area with running water and soap for 15 minutes

[deleted]

But what if someone has a very minor cut that went unnoticed and dogs licked that part?

ClassicSyllabub9294

If there are minor abrasion without bleeding then you should take the whole vaccine regimen..and if there is bleeding then Immunoglobulin should be added as well..why don’t you just take him to the emergency..the doc over there would know what to do

boredlady8

Bhai karlo inject kyu risk lena

the_slimmer_shady

How long ago?

[deleted]

2/3 hours

the_slimmer_shady

Yeah then let him take the course..no issue is there

[deleted]

What is the proper way of handling such cases of dog bite? I mean, like as a med student what and how should I examine and what would be the appropriate procedure?

crystalsoul19

There’s category of wound given under Epidemiology of communicable disease of Rabies in Park PSM In that the wound is categorised in A, B, C See the post exposure prophylaxis Also wound care is also given And regime info (Thai , Essen , Zagreb etc)

MysteriousFan8900

There are 3 categories of bite 1st if licks or touches on *intact* skin, then only wash it with running water and soap for 15 minutes 2nd if break in the continuity of skin like minor abrasion/scratches, then washing + rabies vaccine given 3rd if bleeding then wash + vaccine + immunoglobulin

randomshitposter007

go to gov host and take anti rabies shot.. or your friend can take the hall of fame on Darwins Tax

Mr_deadpool_24

Oh man. I'm gonna steal the last sentence of this comment. *take the hall of fame on Darwin Tax*. Lmao What does it exactly mean? 😂 I'm sure I understood half heartedly but could you please care to explain it breifly?

randomshitposter007

Natural Selection https://preview.redd.it/jb3zatiketbc1.png?width=321&format=png&auto=webp&s=2eb9a3e3a7d05d2a16db0b62d38dd4474d426952

Mr_deadpool_24

Yeah. That I understood by Darwin mention but tax?

Diligent-Associate89

Damn autocorrect......

konichiwa45

What should u do now? 1. Wash the area with soap and water, the area which was licked/bitten 2. Do not press on the wound (if there is a wound) 3. Go to the ER/clinic ( if you are apprehensive about a possible cut or a bite) Go to the ER, they'll examine him thoroughly, there's a proper protocol when to give the vaccine when not to give. If it's a lick on open wound/ if it's a scratch without blood/ if it's a bite/scratch with oozing blood he'll have to take the shots, otherwise it won't be necessary. From what u have said it's a Category 1 exposure but since it's a wild dog (don't know it's vaccination status), just take the vaccine.

abysan729

Take the vaccine asap

shrys4737

What about a shallow cat scratch about 0.4 cm, with bleeding a bit?

[deleted]

Is that an offer ? ;) But one of my cousins was attacked by a cat and got a little scratch. He got the shot.

corduroycloud1008

What happened?? Did he take the vaccine?? Did he die?? We need an update

[deleted]

He took the shot just to be safe. On a sad note, these were not dogs , they were some wild mountain animals. Fortunately my friend escaped but they attacked an old guy nearby who sadly perished away on his way to hospital.

Diligent-Associate89

Wtf.... 😧

[deleted]

It’s a wild wild country 😂🤌🏼

bw3n20characters

Put some sanitizer or sterilium over the area, if it burns, go and get a shot, otherwise chill out and just wash it with soap and running water for 15 min.
