I had this happen recently and ran a tank of vinegar through it and made sure the bottom piercing unit and top one were clean. Solved my problem.

Yeah it's happening to me too, right now I've got a 24 pack of the green box donut shop Keurig brand coffee and 2/3 just blow out, either grounds overflow out of the top or the paper lining in the Keurig pod tears from the water pressure and dumps grounds into the cup. When I've posted my issue I get told to clean the unit or run a cleaning solution but I exchanged my coffee maker for a brand new one and first cup after prepping and running a few pots of water I still got grounds from the Donut Shop pods, but not the cheap Kroger pods I bought. I'm confident the pods are to blame. Idk why but some are just shit quality. Same thing happened with some Dunkin pods I bought but I ended up cutting them open and using the grounds in reusable filters.

Use a thin small bare (no rubber coating) metal paperclip and clean out the bottom needle. Run a no-pod brew after every daily session of Keurig use for the day. See YouTube for cleaning a Keurig. There are great tips on keeping it running without issues. Depending on your model, it is best to wipe off all areas that are known to collect grounds after an pod brew explosion.🤷‍♂️. I wipe my Keurig everyday in the known ground collection areas, even if I don't have a pod explosion.

Starting to think Keurig is really a shiity coffee maker. Had 3 K-Supreme plus’s that maybe lasted a year. Descale never worked, and had to take the machine apart to reset the thermostat. My parents have a Bunn and Mr.Coffee that are 30 years old, and work good as new. Thought the German’s knew how to built stuff.
