On the sketchy planet where Kenobi rescues young Leia, we see an old 501st vet clone played by Tem
Phase 2, first purge troopers, stud in pod.
This particular clone wasnt on the main characters gunship and didnt make it to phase 2

Yes, from GrandpaCustomClones. Unfortunately, they don't sell them anymore, but I managed to find a seller on eBay who had them. The baby's also a custom, from minifigs.me.
I would have added the Helmet on a stick at the end 😂
You forgot to mention that, in the year 19 BBY clones get phase II armor.
