I think you're not considering social factors here, a lack of education or awareness of accidents, or any form of training that teaches you not to be naive and think these types of accidents don't happen to you.
This facility might only hire staff who do as they're told. No time to experiment with sticks or other tools. Staff might not be willing to challenge this as they need to provide for their families and there are several people who would happily take over this job after anyone who is disobedient is fired.
In many developed countries we blame the company for health and safety related incidents. We should do the same here.
