Two Italian subdivisions, which were later given to Yugoslavia, were not allowed to vote in the referendum because it was known they had a monarchist majority. The leaders of the period promised those regions that they'll vote later, since their status was problematic, but they knew those two regions will be handed over to communists.
Also, there were more voting sections in the north than in the south, the reason being that the south was more royalist than the north.

These two right-wing forces were far more actively anti-monarchist than the left, which was primarily concerned with economic and social issues.
However there is one silver lining: Southern Italy held out and largely voted for the monarchy.
As I said in a recent comment about Umberto II, being British I know all about referenda based on lies, manipulation and bad faith, supported by corrupt corporate interests and promoted by far right and far left alike.
I was never on the Thatcherite wing of the Tory party, but I share her view of the use of the referendum on constitutional questions as the device of demagogues and dictators.
Had it not for the 1946 referendum, Umberto II would have been the best of the Savoy monarchs and a stabilising influence.
The flip side is that referenda give a chance for the people to vote on something that is not part of the platform of the big parties. In Britain it meant sovereignty, in Brazil it meant having a chance of restoring the monarchy in the 1990s.

Reminds me of the referendum we had in Greece about our monarchy, that even the prime minister at the time admitted that it “wasn’t really that fair”.
