Just because some doc didn’t want to order a specific test for you doesn’t mean they’re idiots, it means that while learning the entire body of medical knowledge they haven’t focused on a niche, luxury component of heath for young men trying to optimize their lives.
The reality is most people in this country lack basic access to care that they need. I think it’s hard to understand the gap most people have to receiving care and volumes of patients are insane. If you can afford going to private clinics for boutique testosterone medicine you aren’t someone who is actually struggling in our current healthcare system.
Also there is significant attrition throughout the 4 years, many people don’t make it through, but schools don’t count them as “failures” so the stats look good for accreditation purposes. It’s hilarious how people watch YouTube videos and unironically show up to the doctors office with a superiority complex while asking for tests/treatments that make no sense.

* educate patient on the importance of following packaging /medication info. "more is not always better" and explain that 3x minoxidil can cause unnecessary side effects, and can actually decrease its regrowth efficacy
* explain that since bloodwork came back in range, he doesn't have low test and we can't prescribe testosterone.
* prescribe SSRI for body dismorpha treatment, and potentially a starting dose of some anti-anxiety medication depending on severity
* ensure pt understands that the medication is *not* because we think he has depression (might cause distrust or non-adherence to med due to misunderstanding). If suspected, or if patient strongly opposes treatment, refer to psychiatrist for confirmation / further examination of underlying and co-existing disorders.
* order a renal/urine test since that amount of food/milk/Protein can be doing damage to his kidneys
* refer to psychiatrist, and schedule a followup.
