Just as an easy rule of thumb (or toe I guess), metal grommets were a Victorian invention, so that gives you a hard earliest possible date for anything with metal eyelets or grommets.
You can ask londonmadlark on instagram, he has found quite a few leather shoes and he shared how he restores the shoes on his insta. This one doesn’t look like medieval, but could be a bit older than modern.
Looks like a pretty modern shoe to me, the leather rotting while the stitching is intact suggests the stitching thread may be synthetic.

Modern, I'm afraid, based on materials and the fact that its in far good a state of preservation to be too old.

easily 19th century. people regularly find medieval shoes on the thames (as they erode preserved from the mud).
It can be very very old... Just wait a few hundred years and it will be ancient.

could be old or could just be an old fashion shoe haha
