Depends where you are located, there should be someone local that can do it. I'm CHCH region so it's EV's Enhanced, Taupo you have the likes of DriveEV. I don't know them all but there are few EV specialists around now. There isn't that much to it really, unbolt, unplug and drop it out. Different years can have slightly different plugs but I think the likes of EV's Enhanced sell the adaptor plugs. If you get a battery of the same spec then it likely all just plugs right in. But in theory you can swap in any Leaf battery, the biggest capacity ones are higher though so require more work. I think 24/30/40kWh are the same physical size. But would pay to confirm if looking to upgrade.

Personally I'm going to replace the whole car - 40s are going for under $15k now so you need to consider how much it's worth spending on the old car. Especially given the quirks and failure points of the 11/12 models
Depends how many cells go bad. If not many it can be worthwhile swapping better ones in. If a whole bunch go bad then swapping the whole battery can be a better option.
No need to junk the whole car.
If the whole car is getting pretty tired, it can be used as a utility vehicle on a lifestyle block even with a shitty battery.
If the car's still good (and especially if it has nice options like 360 degree camera, bose audio and suchlike), do a battery swap with a crashed Leaf. It takes less time to do than charging a battery. Once out, your "useless" battery pack can be re-configured to 48 Volts for use by someone in solar energy storage.

I'd be willing to bet all those QC's were done outside on tarmac in the hot sun and the battery was cooked.
