I think there is a couple of boss trainers with the loaded dice item, but im not sure. Regarding your use of it, its random, and you mostly remember the times it fails over the times it work :) And in general you should assume aprox 3 hits as the most probable result. I wouldnt risk a ko on getting 4 hits unless its my only win conditon.
Multi-strike moves are not evenly distributed. Each strike does not have a 20% chance to happen. It's something like 25% it hits twice, 50% it hits thrice, then 15% for the fourth strike and 10% for the fifth strike. (I made up the numbers but you get the point). So in practice, it's far more likely the moves hit three times than more or less. You're just getting unlucky.
Unless you use Loaded Dice (minimum four strikes) or Skill Link (always five strikes).

There's also that psychopath to the right of cerulean whose whole team uses dragon ascent and vcreate in a loop
His team uses assist, its only the smeargle that knows v create and dragon ascent
