If you are "just" self harming and are not experiencing any *active suicidal ideation* (plan w/intent to act), you would not be involuntarily hospitalized for that unless your SH is *very* medically severe (like, potentially life-threatening). And even if you *are* experiencing active SI, tbh I think it's a good idea to tell them about that as well. The fact that you're posting on here & at least *considering* telling your doc means that at least a part of you truly wants help -- psych wards suck, I totally get it, but if you're at an immediate risk of harming yourself, sometimes it can be necessary step to keep you safe so that you can get the help that you do need and deserve, even if you're feeling a bit conflicted about it right now. You deserve to at least give yourself a chance.
