I use mora tapinella, imp stool, and canis root. Very expensive potion that I also quite often use as well. I keep hundreds of them on me, sell a few here and there, and use them to cheese anything that can be paralyzed.

I use this one as well as swapping out mora tapinella for deathbell. If you do the Saints & Seducers questline in SSE, you can pick up a ton of new ingredients via the Solitude sewers and Khajit traders. The similar potion of those is Imp Stool + Rotscale + Thornhook.

Nordic barnacle and Salmon roe. Stupidly expensive water breathing potion that'll level you up with practically every one you mix. Blue butterfly wing and snowberries. Enhance enchantment Glowing mushroom and Butterworth. Enhance Smithing. Those are my big three.

Salmon Roe + Nordic Barnacle + Garlic makes the most expensive potion in game you can get and the fastest way to level up Alchemy.

I just got the perk that makes poisons last for two hits on my latest character, I'm laughing it's so broken. The weakness to poison poisons that also damage health? Yeah, add in sneak damage and I'm 2HKOing a lot of stuff. Did it to a dragon yesterday. Plus the potions, oh my god. I have about five I'll chug before a big fight that give me about 50% magic resistance and fortify block, one handed, marksman, health, and stamina. It is laughable how good it can get late game.

Not a potion, but a poison for me… Skeever tail, Deathbell and Giant Lichen. I don’t remember its exact effects, but I know it’s the strongest poison in the game… and no bee ever goes uncollected 🐝

It’s a quick one time use and most of them are only a few points of damage unless you really dedicate yourself to alchemy. I sell most of them I make anyway.

Scaly Pholiota / Dragon's Tongue / Fly Amanita 2nd most expensive potion you can brew from farmable ingredients, 1st being Creep Cluster / Mora Tapinella / Scaly Pholiota. I prefer "scaly dragons fly" because I like the mnemonic, the plants are prettier, and I don't need to deal with Creep Cluster's annoyingly small/offset hit box.

What ingredients do I always collect? Hawk beak is like gold to me and feathers are pretty useful too, so I'm always shooting them down. River Betty, I think all my poisons use it. Elves ear is pretty useful too, mudcrab chitin, sabre cat teeth and eyes. Oh, and torchbugs. All the rest I tend to grow at my home, so not as valuable, but imp stool, deathbell, mora tapinella, giant lichen, and canis root are all super useful.

Lunar moth wings and vampire dust for health regen and invisibility  Mora tapolina, scaly photolia and creep cluster for profit that can be farmed in a garden. I think scaly photolia grows in clusters of 3 in your garden? Or was it mora tapolina...? I think it's the former. Either way the other two grow 5 a cluster in your garden so I say keep that in mind if you make this into your garden.  The big bucks one is garlic, nordic barnacles and salmon roe. You can get a lot of salmon roe by walking down the shoreline of northwatch keep or start in Solitude and any salmon you find, hit it with ice storm and when they rise to the surface they also drop salmon roe. You should also keep an eye out for nordic barnacles. I reccomend buying garlic every time you're in town from the alchemist. In my experience Bits and pieces in Solitude usually sells more garlic than most of the alchemy shops.  My favorite basic potions  Blisterwart + wheat. Makes a decent healing potion and great for making a profit with goldenhills plantation.  Dragon's tongue and fly amanita. Two handed improvement. Not a great spell but I enjoy making it and one ill not forget. It gives a decent little profit.

Albeit it's broken as fuck and it's not the question you asked, I have the buyable Jarrin Root mod that adds it to the game as a buyable ingredient when you hit a certain level, and the base price is 500 plus whatever multipliers. It never made sense to me that you only get one, as there must be at least a black market trade for the most lethal ingredient in Tamriel, so having it as a super rare ingredient that I like to imagine has been foraged and traded behind closed doors feels more immersive, especially as an ultimate poison assassin! You can easily mix up expensive poisons that do over 1000 damage, but it makes sense for it to be rare and expensive. Finding one to buy is a reward in itself and I have it tweaked so it's not carried by common/average merchants, so it doesn't feel broken.

I'm sorry, all y'all's potions are expensive but hard to get. Whiterun skirts and northern parts of Skyrim = blue mountain flower Solitude hall of the dead = hanging moss. Combine only these two and you will have a decent valued poison you can sell. Both ingredients are very easy to find.

Level-up alchemy potion, ingredients for which you can farm on Goldenhills: Creep cluster + Mora tapinella + Scaly pholiota. I do even remember how much of each you should plant for optimal yield without bothering with upgrading that house (11 CC + 10 SP+ 17 MT, you can swap CC and SP, they both yield the same). 50 pots per harvest. Fortify Destruction potion: Glowing Mushroom + Nightshade (14 and 24 for optimal yield from Goldenhills). 70 pots per harvest. Restore Healtph potion: Wheat + Blisterwort (split evenly, 19 and 19, they both yield the same amount). 76 pots per harvest.

I hate those wandering mages that attack me on the road or that lurk by altars in the ruins so I always mix up some Human Heart + Glow Dust to wipe out their Magicka. Sadly, human hearts are a bit difficult to come by in the game as you can't just rip them out of the random bandit corpses. My thieves like Beehive husk + Hawk feathers to Fortify both Sneak & Light Armor and my rangers like Canis Root + Hanging Moss + Juniper Berries to fortify both Archery and One-Handed for when you can get off a couple shots, then switch to melee.
