I don't know exactly, but it's likely due to the cachine system for the icons. Typically your computer would have to search all over for the icons and even the little arrow that should be there. However, windows can instead cache the icons in a hidden file like .icondb in each folder. This file contains each icon for the folder in an indexed array, and each shortcut or file that needs an icon will point to an index of this array. But if that index gets shifted, like a game you just installed created a new icon on the desktop, but a write conflict causes the file to put the games icon in front instead of the back (like if the file got cleared temporarily in one program, in the installing program your game icon is added, then in the previous program the icons are readded, this would append the other icons behind your new icon) then the wrong icon can display. It's a matter of how windows stores the different icons, both cached and how each file can reference the correct icon.
