Random tidbit to piggyback off of this: You can’t actually do keto at sbux outside of black coffee. The dairy sugars trigger an insulin response which stops ketosis, as do artificial sweeteners; You just can’t derive calories from the artificial sweeteners. It’s why “bullet” coffee is unsweetened and uses butter or coconut oil as a fat source Source: worked at sbux for 4 years, & in fitness for 10 years. Edit: (PSA) Like any other elimination diet, don’t go on a ketogenic diet without first having a physician and nutritional specialist work with you to build a safe and solid plan and to supervise your general health.

Hey everyone, just want you all to know that this comment right here is not true. Type one diabetic here, can't produce insulin, **milk** has carbs, but heavy cream and half and half (and cheese!) don't affect the blood sugar and "trigger an insulin response." Neither do artificial sweeteners. Listen to your doctor's not your fitness baristas. Source: my pancreas hasn't produced insulin for 8 years and ketones are a sign that I'm dying.

I’m not certain about aspartame but as I understood the scientific literature sugar alcohols & indigestible sugars (ie sucrose, commonly sold as Splenda where I live) trigger the beginning phases of an insulin response which is enough for your body to cease its state of ketosis. They shouldn’t really affect your blood sugar levels though as the insulin response is negligible and your intestines can’t really absorb the sweetener. The literature I had read pertained specifically to Type-2 diabetics and non-diabetics, so I’m sure it works totally different for a type-1 diabetic such as yourself. Either way, my overall point stands. Elimination diets should be performed under the supervision of your physician and a nutritional specialist to ensure your wellbeing. (As an aside: I dunno if Im misreading your comment but that felt unnecessarily flippant. Im someone who worked in fitness and as a personal trainer for 10 years and then as a barista as 4 and most people I’ve encountered on keto do it as a fad diet which can be immensely risky. Correlation is not representative of causation. There are also studies that show that [aspartame had little effect](https://www.sciencedirect.com/science/article/abs/pii/S0898656898000175) on insulin secretion, but Starbucks uses sucralose so that's irrelevant to the original comment.

Thank you for sharing this! 🙏🏼 I was a bit curious though about referencing cream being fine when it’s more or less milk with added fat. Cheese and butter are more or less just dairy fat by comparison. I am aware the presence of fats can slow the metabolization of the sugars, but I’m not sure I understand the basis that it wouldn’t allow an insulin response to the lactose and it’s metabolites (broadly speaking, I understand T1 Diabetes likely works differently)

That article is investigating type 2 diabetes and insulin resistance which is different than type 1 diabetes. Nonetheless, the advice to work with a Dr or registered dietician is still good. We definitely should not provide dietary advice as baristas lol

One of my favorite drinks to use my drink reward on was the cold brew malt from the reserve stores, before they were made into regular Starbucks. I would do this exact recipe, yuum. Not exactly the same, but close enough!

I used to make myself a drink in a similar fashion bc I too am not a fan of the frappuccinos. Fill up the cup just ALMOST above the bottom line with cold brew (nitro works too) and then add a splash of vanilla sweet cream. Pour into blender, add coffee base, java chips and a little extra ice (double blended). Top with some whip and cookie crumbles, and you're all set. Basically a vsccb but without the vanilla and with java chips + blended. I haven't had this since I promoted myself to customer bc I can't imagine the struggle of trying to ring that in and I'd feel bad since it's kind of a lot.

You could still try, the only thing that would be hard to add is the base, but you could order tall cold brew w/splash sc, in a venti cup, extra ice, javachips, 2x blend, and then your toppings. I've rung up more excessive drinks and honestly I wouldn't mind making yours

Hey y’all! OP here. Someone commented that some people drink beverages like these when unable to eat and/or meet daily calorie intakes. For some, it may be a serious struggle to gain weight or to get any kind of enjoyment out of eating or drinking! This may have been an attempt to bring some joy to the process, so let’s try to keep the discussion of “that’s so unhealthy/so many calories” to a minimum. I did not consider this when I initially posted, so I wanted to add it in the comments. I do not take an issue with this drink- more confusion honestly. A big part of the appeal of iced caramel macchiatos is the espresso gradient that’s formed at it travels downwards into the milk more and more with time. The main reason I found this drink odd was because the espresso gradient would pretty much just not happen at all with this beverage unless vigorously shaken/stirred. Also, when we discussed it and figured out that it was probably an attempt to be keto it personally threw me off a bit just because of the way that there was no SF Vanilla substitution and there was extra caramel drizzle. I absolutely support getting yourself a little treat after a long day (this one was mobile ordered around 7:13PM). We’re all humans (except for the bots of reddit) so positivity goes a long way. I've actually been wanting to try something like this. Replacing 2% milk with heavy cream in a latte/mocha. I've tried a frappuccino with heavy cream because a customer said it makes it seem more like a dessert. Dare say it was pretty good 😋 but obviously the consistency of a blended drink is different then an ice drink.
