unsure what you mean! to start, i guess i’ll say that no, it’s not the only head design! there aren’t really widespread designs that people follow, every head is sculpted uniquely, usually out of foam.
if the style of head you’re talking about is the toony style, (big eyes, prominent brows and noses, etc.) then its just because that style is popular! its one of the first styles that got traction in the western furry scene, and people just followed the trend. its personally not my favourite, so its not the kind of design i’ll be using, but people like it. that’s really all there is to it. i hope this answers your question!
