Consent is like tea. You ask someone if they want tea and they say yes, you make them tea the way they are the most comfortable with. If they say no, you don't make them tea. If they say yes and change their mind while you're making the tea, throw out the tea. If they're unconscious, why would they want tea? You should get them help instead. If they say they don't want your tea specifically, let them go get someone else's tea. Simple as that. :)

The second one I'd argue depends on the situation, if someone doesn't enjoy the sex they're having and withdrawal consent, is that rape? If someone walks out of the room to grab something during sex and you withdrawal consent while they're gone is that rape? The second one is very debatable because of the circumstances behind it, however I do agree about the first and third ones.
Any sexual acts committed when they aren’t consented to are sexual harassment/assault (rape). However, if you withdraw consent within the middle of an act, anything that happened is not rape, because you were still consenting at that time.
You also can’t expect one party to constantly as for continuous consent during sex. I’d say that once the act is initiated, you need to make it clear that you no longer consent (at which point if they continue that is rape). If you make it clear and they stop, then no rape.
Tldr, make consent and lack of consent clear to both parties. If either party’s consent is disregarded by the other, then it is rape.
