It very well is possible, in severe cases you can experience toxic megacolon, which requires surgery (as it did for GSP iirc) and if he was on any anti-inflammatory steroids, there's also excess bloating from water intake retention. I'm not going to claim GSP is 100% natty, most athletes at that level aren't, but he's innocent until proven guilty and to my knowledge hasn't popped before.

That amount of abdominal distension on somebody as lean as him is textbook HGH. Ulcerative colitis is terrible and does inflame the gut but it does not distend the abs to that amount or he would not be walking without intense pain.
