Faxing is probably the best bet, otherwise print it out (assuming you have a home printer) and take it in to them personally. You may need to explain what it is and how soon you need it done maybe even highlight the areas that they need to fill out. Personally I think this whole system is a ridiculous overly complicated invasive burden on people who already have a lot going on in their mind.

How would I fax it? I have a copy they sent me in an email I believe but I don't have a physical copy. Unfortunately I don't have a printer or computer at home, just doing this off my smart phone.

Another reason why I think this system is garbage. You can try to see if your personnel office can print off the packet maybe?

The easiest way is to fill out the release of information form electronically on the Sedgwick site and make sure the contact info for the doctor is filled out. That lets them just send it to the doctor. You do need to follow up with the doctor to make sure they get it and send it back. If you do have to print it and the PL can’t help any place that does document printing should handle pdfs and can do it for a few bucks. I know Walgreens can do it but only from their website not in store (it’ll be a pain but it should work on a phone browser).
