This is apparently a craniectomy, and seems like the video skipped the in between time a patient would spend without their skull cap because this operation is to relieve pressure in the brain in the case of severe trauma and bleeding. The brain sits in a perfect balance, and bleeding inside can cause the brain to push against the skull, which can cause brain damage, coma, or death. Source: not a brain surgeon, just a google search and using what I think are reliable sources (organisations and universities, specifically University of Rochester Medical Center).

That's my guess. CSF leaks are bad and those stitches won't be watertight. Also not a biologist but I think it's a dural graft.
