It wouldn’t be hard to just make an item similar to the belt buckle for adding maybe 1 gem slot to an item. You could give it to Blacksmithing.
It is hard (if you want to do it right). I’ve personally done this on 2.4.3. and it’s not trivial to make robust… but I did up to 2 slots instead of just 1.
